Sauna During Pregnancy: The Core-Temperature Rule

Room temperature is not the risk in pregnancy; your core temperature is, and the concern begins near 39°C. In a pooled review of 12 studies and 347 pregnant women, none crossed that line, and sauna sessions averaged 37.6°C. Keep sessions shorter, cooler and hydrated, and ask your clinician.

The number that matters here is your own core temperature, not the one on the sauna’s thermometer. Teratology research puts the concern at sustained maternal core temperature near 39°C (102.2°F). A hot room is one route toward that line, and by measured data a slower route than most people assume — but the line is the thing being managed, and every practical rule follows from it.

Why does the room temperature tell you so little?

Because a room only heats you at the rate your body fails to shed heat. In a hot sauna, blood moves toward the skin and sweat evaporates off it, carrying heat away. Air temperature is an input to that balance, not a dose you absorb. Eight minutes at 90°C can move your core less than twenty-five minutes at 70°C, and a bather who steps out to cool between rounds resets the whole calculation.

That is also why humidity matters more than the wall gauge suggests. Sweat only cools you when it evaporates, so a heavy löyly (the burst of steam from water on the stones) briefly makes a given room harder to shed heat in. Keeping the löyly light is not superstition; it is the same lever as sitting lower or staying less time.

How far does a normal sauna actually move core temperature?

Less than the caution implies. A 2019 systematic review in the British Journal of Sports Medicine pooled 12 studies in which the core temperatures of 347 pregnant women were measured during exercise or passive heating. No woman exceeded 39.0°C. The highest single reading, 38.9°C, came from land-based exercise — not from any hot room.

The authors’ own conclusion was that sitting in a 70°C sauna at low humidity for up to 20 minutes kept the women studied under the teratogenic threshold. Read it for what it is: a synthesis of thermophysiological measurements in a few hundred women, not a study of birth outcomes, and not a substitute for the advice of someone who knows your pregnancy. It is evidence about the mechanism, and on that question it is the best evidence available.

Why do hot tubs come up in the same breath?

Because of one American study, and the honest answer is that it did not convict the sauna. Milunsky and colleagues followed 23,491 women and reported an adjusted relative risk for neural tube defects of 2.8 with hot-tub use (95% CI 1.2–6.5). The sauna estimate in the same analysis was 1.8, with a confidence interval running from 0.4 to 7.9 — an interval that wide settles nothing at all.

Mechanically, immersion is the more plausible worry: skin under water cannot evaporate sweat, and a tub is something you sit in rather than step out of. Yet the measured data above shows a 40°C bath raising core temperature less than a 70°C sauna. Those two evidence lines do not resolve neatly, and nobody should pretend otherwise. What they agree on is the behavioral rule underneath: keep any heat exposure short, and leave before you feel hot rather than after.

What do Finnish women change, and what stays the same?

Finnish practice treats this as an adjustment, not a suspension. In the registry study by Saxén and colleagues, 98.5% of the mothers examined had used the sauna regularly, and no difference in sauna habits separated mothers of affected infants from the referents. Suomen Saunaseura, the Finnish Sauna Society, states the national consensus plainly: customary bathing in pregnancy carries no known danger (in Finnish).

One thing is not a moderation but a starting condition: if the sauna was not already part of your week, pregnancy is not the moment to begin. An accustomed bather sweats efficiently and knows her own signals. That self-pacing, not the thermostat, is what keeps the number under 39.

What belongs in the conversation with your clinician?

Everything above is context to bring to that conversation, not a substitute for it. Bring the specifics: how hot your room runs, how long you sit, whether you were a regular bather before, and anything your pregnancy is being watched for — hypertensive disorders in particular. Caution is widest in the first trimester, and that is where clinical opinion outside Finland diverges most from Finnish habit.

The ordinary contraindications do not soften here either. Is a hot tub riskier than a sauna during pregnancy?
The evidence does not settle it. In the American cohort that raised the alarm, hot-tub use carried an adjusted relative risk of 2.8 for neural tube defects (95% CI 1.2–6.5), while the sauna estimate spanned 0.4–7.9. Yet measured core temperatures rose less in a 40°C bath than in a 70°C sauna.
How long is too long in the sauna while pregnant?
No official limit exists. The best pooled review of measured body temperatures put roughly 20 minutes at 70°C and low humidity inside the safe range for the women studied. Finnish habitual practice is shorter still, and self-paced: leave when you feel like it, not when a timer allows it.
Does throwing more löyly make the sauna riskier in pregnancy?
Humidity slows evaporative cooling, so a heavy löyly makes any given room temperature harder to shed heat in. That is a reason to keep the water light and the session gentle, not a reason to skip it. Sit on a lower bench, where both the heat and the steam are milder.
